When investigating patients with epilepsy, MRI is the tool of choice. The advent of high-resolution MRI with specific seizure protocols has positively impacted the treatment of these patients by increasing the chances of determining the underlying etiologies. This course covers the role of MRI in the diagnosis and management of patients with epilepsy. It includes a discussion of the protocols and their limitations, with emphasis on MRI in the differential diagnosis and presurgical evaluation of the disease. The clinical evidence supporting the application of many anatomical and functional imaging methods is also described. The course concludes with an assessment of future developments, including new technologies and potential new applications of MRI for patients with epilepsy.
